RISE team
RISE works as part of the Ontario Health-led implementation supports to Ontario Health Teams and their Primary-Care Networks (PCNs). RISE brings together a distributed group of RISE team members and draws on McMaster Health Forum staff. RISE team
Our team includes two co-leads (John Lavis based at the McMaster Health Forum, and Rob Reid, now retired and formerly at Trillium Health Partners’ Institute for Better Health) and one associate lead (Brianne Wood based at the Centre for Social Accountability at the Northern Ontario School of Medicine). The team also includes a RISE leadership coach (Bill Tholl) and two RISE coaches (Mike Hindmarsh and Leslie McGeoch), as well as the Forum’s managing director (Kaelan Moat) and senior scientific lead for evidence support (Kerry Waddell).
