Interested in learning from Ontario Health Team (OHT) leaders, from RISE and its distributed network of experts, and from other partners in the OHT Central Program of Supports, and from the Ministry of Health? The events in this series (many of which are webinars) are designed to support OHTs to learn and improve rapidly both in using a population-health management approach to move the needle on quadruple-aim metrics (e.g., care experiences and specific health outcomes) for their year 1 priority populations and in putting in place the eight OHT building blocks.
