RISE team

RISE works as part of the Ontario Ministry of Health’s OHT Central Program of Supports to support the development and maturation of Ontario Health Teams. RISE brings together a distributed group of RISE team members and draws on McMaster Health Forum staff. RISE team

Our team includes one co-lead and one focal point based at each of the McMaster Health Forum (Forum), and Trillium Health Partners’ Institute for Better Health (THP), as well as an associate scientist and one focal point based at the Centre for Social Accountability at Northern Ontario School of Medicine (NOSM) University. The team also includes RISE coaches, as well as the Forum’s managing director, a senior project manager, and an engagement coordinator.

The RISE focal points provide leadership for specific RISE programs focused on implementation supports for OHTs (e.g., coaching to support population-health management, online peer sharing and learning sessions) and – depending on the topic area – for specific demand-driven activities (e.g., RISE jamborees) and products (e.g., RISE briefs and rapid syntheses), as well as resources that support OHTs and OHT support partners (e.g., content for the RISE website and monthly RISE newsletters).
