RISE works as part of the Ontario Ministry of Health’s OHT Central Program of Supports to support the development and maturation of Ontario Health Teams. RISE brings together a distributed group of RISE team members and draws on McMaster Health Forum staff. Our team includes one co-lead and one focal point based at each of the McMaster Health Forum (Forum), Ottawa Hospital Research Institute (OHRI), and Trillium Health Partners’ Institute for Better Health (THP), as well as the Forum’s managing director, a senior project manager, and an engagement coordinator.
The RISE focal points provide a single point of access to RISE for questions and requests for resources from:
- OHTs, with each focal point assigned to teams in specific regions (Leslie McGeoch for central, Anna Dion for east and north, and Kerry Waddell for Toronto and west)
- health-system partners that are supporting OHTs, with each focal point assigned to partners based on their alignment to specific priority populations, OHT building blocks, and sectors
They also provide leadership for specific RISE programs (e.g., coaching and collaboratives to support population-health management, online communities of practice, and webinar series) and – depending on the topic area – for specific activities (e.g., RISE jamborees), products (e.g., RISE briefs and rapid syntheses), and resources (e.g., content for the RISE website and monthly RISE newsletters).
