OHT Impact Fellows- Lunch and Learn Series: Using Gap Analysis to Inform the Design of Care Models for Seniors

This webinar will explore how gap analysis can help inform the design of care models. Drawing on her experiences as an Impact Fellow at Scarborough OHT (SOHT), Charlotte Anderson will share lessons gleaned from implementing programs of care for seniors requiring mental health and addictions services. She will present key findings and engage SOHT staff in a conversation about the role of gap analysis in program development. Specific goals of this webinar are: 1. Review key characteristics of programs of care and the use of a gap analysis in the development 2. Discuss questions to pose, stakeholders to include and important considerations when implementing programs of care for Seniors requiring mental health and addictions services within OHT's. 3. Discuss the role of a gap analysis in program development and present learnings from SOHT, and tangible lessons for future development of programs of care for Seniors requiring mental health and addiction services.
