Advancing OHT and PCN leadership: Opportunities and challenges
Join ALIGN and North Toronto OHT and PCN leaders as we unpack the realities of leading collaboratively across PCNs and OHTs. Together, we will explore a practical framework for understanding these challenges and identifying ways forward.
This opening session situates participants within the current OHT and PCN context and lays the foundation for the rest of ALIGN’s Collaborative Leadership Workshop Series. Subsequent workshops will tackle the personal and relational demands of collaborative leadership, then move to the partnership structures, decision-making approaches, and ways of working that support sustained collaboration.
By the end of the session, participants will be able to:
- Define collaborative leadership and decision-making in OHT and PCN contexts
- Identify tensions that commonly emerge when leading collaboratively across organizations and the network of independent primary care providers
- Understand the five conditions of Collective Impact and their application to OHTs and PCNs
- Explore a real-world example of how an OHT-PCN structured their partnerships and ways of working
- Explore the factors and lessons learned that enabled success in the featured OHT-PCN relationship
