Caregivers are essential members of the care team that can support addressing Alternate Level of Care (ALC) stays, improving care transitions, and creating better care models for patients. Pro-active approaches to include and support caregivers in patient care and transition planning are critical across hospital and community care settings. This webinar will highlight leading practices that enable caregiver participation as part of the care team, based on the (ALC) Leading Practices.
