To help OHTs implement a population-health management (PHM) approach, RISE is hosting a series of webinars.  This second webinar builds upon the first webinar and also integrates feedback received from OHTs.

The learning objectives for this second webinar will help OHTs:

  1. redesign care models for their segmented priority population through co-design
  2. understand the key changes needed to improve outcomes for priority populations
  3. cultivate shared learnings by hearing from peer OHTs on their application of population-health management

This session will help support OHTs in achieving the following OHT TPA milestones:

  • Re-designing care for patients in your priority population(s)
  • Helping every patient in your priority population(s) experience coordinated transitions between providers
