Population-health management update
PHM overview infographic
Developed in collaboration with the Health System Performance Network, RISE is excited to share a new infographic that provides an overview of PHM. Key information contained in this infographic includes:
- a definition of population health
- an overview of the three sub-curves of population health
- a definition of PHM
- five key components for implementing PHM.
OHTs and their partners can also use this infographic to access related PHM resources such as webinars. Additionally, new resources will be linked to this infographic as they are developed.
OHT collaborative on spreading care models and change ideas to other populations within your OHT
On 15 September (12 pm – 1 pm), a session facilitated by the RISE PHM coaches for OHT implementation/priority population working group/population-health leads was held. This session helped OHTs to:
- identify strategies and best practices to enable successful spread of care models
- understand how to spread to different levels of their OHT (segment sample to entire segment, segment to other segments in the priority population, priority population to attributed population etc.)
- learn from others through practical examples and lessons other OHTs are learning.
Attendees heard from the South Georgian Bay OHT and the Ottawa OHT, who both shared examples of spreading models to their cancer-screening population. South Georgian Bay OHT also shared how they’re spreading remote-care monitoring for their COVID population to their older adults with complex needs population. Additionally, Ottawa OHT shared their key learnings and success factors for spreading a care model for frail older adults to other segments of their population.
