OHT building blocks infographic

As OHTs begin to adopt a population-health management approach to moving the needle on quadruple-aim metrics for their priority populations, they are also putting in place eight OHT building blocks that will underpin this transformation.

To support OHTs in keeping the big picture of the OHT model in mind in their ongoing efforts to improve care experiences and health outcomes for Ontarians, we have developed an infographic about the OHT building blocks. The top half of the infographic presents each of the building blocks, while the bottom shows how they can be leveraged as they implement a population-health management approach.
