In March 2022, we are coming out with our stories from the ivory tower of university-based evaluation of Ontario Health Teams (OHTs). Evaluators from the HSPN have spent the better part of the past year observing the development of OHTs with an aim to understand:

  1. What factors contribute to the successful implementation and sustainability of OHTs?
  2. What adaptations are OHTs making to improve function/performance/outcomes?
  3. What are the barriers to OHTs’ successful implementation?

Join us to engage in a discussion of these questions as they relate to the development of population health management and integrated care in Ontario with attention to selected foundational elements of the OHT model.
