On 18 June, RISE will host a session (11:00 – 12pm) in collaboration with AFHTO and OCFP focused on building a high-performing primary care system. The specific aims of the session will be to help OHTs and their PCNs:

  • Gain visibility to the new RISE program of supports & collaboration with AFHTO & OCFP
  • Obtain a shared understanding of key terms (e.g., HP PC)
  • Understand key drivers to build a HP PC system and enable attachment
  • Learn through real-life examples of HP PC system development
