Peer sharing and learning session

RISE is offering a pair of virtual sessions focused on PCN/OHT priorities, as outlined in the 2026/27 TPA deliverables and guidance, and aligned with priorities outlined in the Ontario’s Primary Care Action Plan (e.g., supporting 100% attachment of Ontarians to primary care by 2029).

The sessions will focus on understanding the PC landscape, the value proposition of PCNs and the role of clinical leaders in achieving PCN results.

Session 1 peer sharing and learning session (14 October, 12pm-1pm) will support participants to:

• create a shared purpose/vision and values for PCN that achieves the TPA deliverables (e.g., attachment, cancer screening, PC engagement)
• develop approaches to identify local PC leaders and change agents in the PCN landscape
• develop skillsets for PC leaders to align priorities with the OHT including addressing local inequities in achieving the TPA deliverables
• understand the importance of self-reflection in enabling effective PCN leadership.

Real-world examples from peer teams will be shared, and a facilitated discussion will help surface enablers and challenges that will be used as a jumping-off point for session 2.

Session 2 Peer problem-solving session (22 October, 12pm-1pm). Building on the key insights emerging from Session 1:

• RISE will facilitate confidential discussions with peer OHTs/PCNs clinical leaders that are at similar stages in their PCN planning and implementation.
• Participants will share learnings on what worked well, problem solve with peer teams on challenges and gain ideas about how to lead through change as they look to identify other local PC leaders, create a shared purpose/vision and values and align priorities.
• Registration:  please register for session 2 by October 19th
• Audience: Session 2 is only for those leading PCN/OHT PC working groups and their members including those leading PC clinics

Participants can attend one or both sessions. For those attending session 2, it is highly recommended to attend/watch the recording of session 1.
