This Community of Practice session will explore innovative approaches to administering home IV diuretics for patients with heart failure in Ontario. Through recent research and frontline experiences, we aim to better understand the current landscape, including the facilitators and barriers to implementation from various healthcare provider perspectives. The session will feature insights from academic research and Ontario Health Team-led initiatives, followed by an open discussion to foster shared learning and collaboration across the heart failure care community. We’ll be joined by Morgan Krauter, NP lead for the Royal Victoria Regional Health Centre Heart Function Program in Barrie, and Lori Vermeersch, Superintendent, Professional Standards & Community Programs with Huron County Emergency Services.
