Enhancing Health Promotion and Disease Prevention in Networked Primary Care

In order to maximize the effects of the integration of population and public health at the primary-care level, it is important to learn from other jurisdictions that have incorporated similar approaches, as well as from the published literature to understand the barriers and facilitators to networked primary care. The rapid synthesis focuses specifically on this, but in relation to efforts to incorporate health-promotion and disease-prevention services in networked primary care.
