RISE peer sharing and learning session on models of sharing resources in primary care

RISE continues to offer opportunities for all approved OHTs and Primary Care Networks (PCNs) to collaborate virtually through peer sharing and learning sessions focused on ongoing and emergent OHT and PCN priorities, including those established through the work of Ontario’s Primary Care Action Team. On 19 March, RISE will host another session (12:00 – 1:30pm) in its series on creating a high-performing primary care sector. Building on the 19 February, this session will focus on models of sharing resources in primary care. Specifically, attendees will learn through examples from the Kitchener Waterloo area and Missisauga OHTs. This session aims to help OHTs/PCNs to:
- learn approaches to building a team regardless of practice model
- understand models of sharing resources to support small group and solo practices
- hear about methods for enabling team-based care when resource sharing is not possible (e.g., atypical role responsibilities)
- continue to exchange learnings and problem solve with peer OHTs and PCNs on how they’re approaching this work.
