Before and after wave 1 of COVID-19, we heard from Ontario Health Teams (OHTs) the desire to learn more about the resources available to them through the OHT Central Program of Supports. The supports have been designed to assist OHTs in using a population-health management approach to move the needle on quadruple-aim metrics for their year 1 priority populations and to put in place the eight OHT building blocks, as well as to meet the specific implementation deliverables set out by the Ministry of Health. The purpose of this webinar is to hear from each of the partners involved in the OHT Central Program of Supports about their offerings, how they are being targeted at OHTs at different points on the OHT readiness spectrum, and how they can be used by OHTs.
