Identifying the Features and Impacts of Community Health Centres

Community Health Centres (CHCs) deliver integrated primary care and social services and programs, which includes addressing social determinants of health within the community. However, there is a need to better understand how CHCs can be effectively integrated into a coordinated delivery system. This rapid synthesis was requested by the B.C. Ministry of Health to help inform future policy development and expansion of funding related to improving access to high-quality, patient-centred, and community-centred primary-care services.
