RISE resumes peer sharing and learning sessions for all approved OHTs in April
RISE will resume offering peer sharing and learning sessions - virtual collaboratives usually held on the third Thursday of each month from 12-1:30pm for all approved OHTs – in April. These sessions are generally structured around one of two objectives:
- learning from OHTs’ experiences (I12 and non I12) with their efforts to advance OHT acceleration priorities, such as implementing integrated clinical pathways and primary care networks
- helping OHTs to implement a PHM approach to care, by:
- creating an opportunity to share examples of priority projects across diverse OHTs
- identifying commonalities and supporting collaboration
- providing a venue to raise questions not yet addressed and top of mind for OHTs
- creating an ‘all teach, all learn’ environment.
Additional details will follow along with a calendar invite for those leading OHT implementation/priority population/population-health work, and for those leading efforts related to the OHT acceleration priorities.
