Population-health management OHT survey results

In fall 2022, RISE issued its second annual population-health management (PHM) program survey to cohort 1, 2 and 3 OHTs to better understand:

  1. the value of current PHM supports and how they can be improved
  2. opportunities for future PHM supports
  3. PHM learning needs.

Among the key highlights, respondents identified the monthly collaboratives, which bring together OHTs from across the province to exchange learnings, as being highly relevant to their work, and they would like more learning opportunities with those at similar stages of PHM progression.

While there was agreement that there was no need to change the approach to coaching, some shared a desire for more ‘in-depth’ support (e.g., more time) and flagged that there are challenges impeding participation (e.g., capacity/resource constraints).

The results show a higher level of learning needs for later components of PHM implementation (e.g., implementation and reach, evaluation). A higher level of need was also identified across all PHM components for later OHT cohorts, which is expected given they’ve had less time with PHM supports. Compared to last year, cohort 1 and 2 identified lower levels of need (cohort 3 did not have supports at this time last year).

The completed surveys informing this feedback were distributed equally across cohorts, but all respondents are currently participating in the RISE PHM program (coaching/collaboratives).

RISE is currently thinking about integrating this feedback into the 2023 program by providing:

  • more opportunities for OHTs to connect with small groups of their peers (e.g., breakout groups)
  • ‘check-ins’ with current teams to understand if they’d like additional support
  • optional assessments of PHM progression
  • a variety of session topics – some focused on earlier components for those earlier in the process and some focused on later components for those later in the process.

RISE is also examining how to integrate additional thinking from a recent RISE jamboree focused on OHTs in the north into its implementation supports.
