New RISE primary-care ‘deep-dive’ sessions

RISE coaches’ poll of participants in a recent peer sharing and learning session indicated that a majority of OHTs/PCNs were interested in participating in ‘deep-dive’ sessions.

‘Deep-dive’ sessions are interactive improvement sessions to apply learnings of a specific primary-care concept (e.g., process of primary-care attachment) and include work between meetings to accelerate progress and result in the execution of an aspect of their PC-AA plan. These sessions are meant to help OHTs/PCNs achieve their TPA deliverables (e.g., attaching all patients on the Health Care Connect list as of January 2025 by Spring 2026).

The estimated time commitment will vary depending on the complexity of the topic. For example, the deep-dive on the process of attachment is composed of two-one-hour sessions (plus additional work between sessions. Between session work will depend on where you currently are in the work and how much time you have available (your pace is up to you). RISE coaches will meet you where you are in the work.
