How RISE’s coaching supports can help OHTs and upcoming dates for interested cohort 2 teams

Since January, RISE’s population-health management (PHM) coaches have been working with approved cohort 1 OHTs to help them implement a population-health management approach to care. Currently, 25 of 29 OHTs are participating in the coaching program. OHTs have found coaches particularly helpful in the following ways:

  • connecting them to other OHTs with similar challenges to exchange ideas
  • helping them develop driver diagrams/logic models that can inform their plans for implementing  population-health management appraoch
  • challenging them to think differently
  • pointing them to the right resources at the right time
  • helping them to apply quality improvement and health-equity lenses
  • facilitating connections to experts

RISE was pleased to extend these coaching supports to approved cohort 2 OHTs in May. Initial discussion with coaches included: gaining an understanding of where each OHT is in their PHM implementation, identifying learning opportunities, and determining a coaching schedule which works for the OHT’s current level of capacity. 

An onboarding session on PHM core concepts and principles and to introduce the coaches was held on May 20th. A second onboarding session will be held on June 29th(11:30am-1:00pm).
