Leaders from the ministry and OH, as well as Betty-Lou Kristy, Chair of the Minister’s Patient and Family Advisory Council, will share insights related to advancing the OHT model, its foundational building blocks and the path forward.

Audience: All approved OHTs and in-development teams, including leadership, administrative staff, patient, family and caregiver partners, member organizations, physicians and other affiliated care and social service providers.
