Identifying Features of Approaches to Supporting Transitions from Child to Adult Care for Young People with Special Healthcare Needs

Youth with special healthcare needs (i.e., those with one or more chronic conditions that cause some limitation in their lives) who transition into adulthood eventually need to transfer from pediatric to adult care. A recent high-quality Cochrane Review documented that the process of transition from pediatric to adult care is often associated with a deterioration in health, as services fail to adequately meet adolescent patient needs. It is well recognized that there is little evidence on the specific features of interventions that are effective in supporting transitional care. In order to further understand the state of evidence as well as the features of existing transition programs and tools to support transitions from child to adult care, CanChild requested this rapid synthesis.
