Engaging with Patients, Families and Caregivers to Support Ontario Health Teams

Ontario is in the midst of major health reforms, among other significant changes is the creation of Ontario Health Teams (OHTs) in which all healthcare providers will work as one coordinated team. This citizen panel aimed to take the first step in producing guidance for Ontario Health Teams (OHTs) about engaging patients, families and caregivers.
